Report of Death (ROD)
If the deceased died is a Filipino Citizen: comply with numbers 1 to 8.
If the deceased died is a Foreigner: comply with numbers 2 to 8.
1. Four (4) original copies of the duly accomplished Report of Death form;
2. Original and five (5) photocopies of Certificate of Death;
3. Original and a photocopy of the Transit Permit;
4. Original and a photocopy of the Health Certificate issued by the Department of Health stating that death was not due to a contagious disease and/or that the human remains do not pose any danger to public health.
5. Original copy and a photocopy of the notarized Mortuary Certification that the casket contains only the human remains of the deceased or notarized Crematory Certificate that the urn contains only the ashes of the deceased. The Certificate must state that the remains was embalmed/cremated in accordance with state or local laws;
6. Details of transportation arrangements (1 original and 1 photocopy):
a. Flight details;
b. Address and phone number of the consignee in the Philippines;
c. Address and phone number of the next-of-kin in the Netherlands and in the Philippines;
7. Original and a photocopy of the Passport or any other form of identification (Birth certificate/State Identification, etc.) of the foreign deceased;
For Filipinos, original and 6 photocopies of the Passport or any other form of identification (Birth certificate/State Identification, etc.)
8. Payment of € 25 for ROD and €25 for the consular Mortuary Certificate in cash or Dutch-issued PIN/Debit Card (subject to additional €0.10 Dutch Bank Service Fee).

Report of Marriage (ROM)

3. Uittreksel uit de huwelijksakte / International version (4 languages) of Marriage Certificate from the Gemeente. The Marriage Certificate must have an “Apostille” from the appropriate Dutch District Court (Rechtbank)
4. Valid passports of husband and wife, and copies of the passports’ data pages
5. Residence permit of the Filipino national
6. PSA authenticated Birth Certificate of the Filipino national
7. For Divorced, Annulled or Widowed Filipino applicants:
- In case of annulment or nullity of marriage or divorce, PSA authenticated Marriage Contract/Report of Marriage with annotation
- In case of death of the previous spouse, PSA authenticated Death Certificate or PSA authenticated Report of Death issued by a Philippine Embassy or Consulate (need not be in PSA security paper if issued by the Philippine Embassy, The Hague)
8. Affidavit of Delayed Registration of Marriage if marriage was not reported one (1) year after its solemnization (Notarization fee: € 25.00)

The Embassy of the Philippines forwards the Report of Marriage to the Philippine Statistics Authority (PSA) in the Philippines, through the Office of Consular Affairs of the DFA. Per experience, the Report of Marriage in PSA Security paper may be retrieved in the Philippines six months or more after the date of filing of the Report of Marriage at the Embassy. Always keep a personal file copy of the Report of Marriage.

Report of Birth (ROB)
Requirements:
1. Mobile Phone Number
2. Accomplished Report of Birth Application Form (Processing fee: € 25.00)
3. Uittreksel uit de geboorteakte/International version of the local Birth Certificate from the Gemeente. The Birth Certificate must have an “Apostille” from the appropriate Dutch District Court (Rechtbank)
4. Valid passports of parents.
5. PSA authenticated Birth Certificate of the Filipino parent/s
6. If parents are married, specifically at the time of birth, PSA authenticated Marriage Contract or PSA authenticated Report of Marriage issued by the Philippine Embassy or Consulate in the country where the marriage took place (need not be in PSA security paper if issued by the Philippine Embassy, The Hague)
7. If parents are not married, the child may use the surname of the father “if their filiation has been expressly recognized by the father through the record of birth appearing in the civil register, or when an admission in a public document or private handwritten instrument is made by the father.” (Section 1, Republic Act No. 9255, 24 February 2004).
Based on Administrative Order No. 1 Series of 2016 of R.A. No. 9255 (An Act Allowing Illegitimate Children to Use the Surname of their Father, Amending for the Purpose Article 176 of Executive Order No. 209, otherwise known as the "Family Code of the Philippines"), the following documents shall be filed at the Local Civil Registry Office (LCRO) or Philippine Foreign Service Post/Embassy or Consulate for registration:
1. Affidavit of Admission of Paternity (AAP) to be executed by the father (Notarization fee: € 25.00)
2. Affidavit to Use the Surname of the father (AUSF) to be executed by the mother or guardian (Notarization fee: € 25.00)
*AAP and AUSF should be personally presented and signed by the concerned parent before a Consular Officer. These Legal Instruments will be processed through JURAT that bears the official seal of Post, and registered in the Registry Book of Legal Instruments. A Certificate of Registration will be issued by Post to the applicant (two Legal Instruments in one Certificate is acceptable).
8. Certificate of Registration for illegitimate children (Notarization fee: € 25.00)
9. Affidavit of Delayed Registration of Birth if birth of the child is not registered within one year from the date he/she was born (Notarization fee: € 25.00)
10. Residence Permit(s) of the Filipino parent(s).
11. If parent(s) is Filipino under RA 9225 (i.e. by the reacquisition or retention of natural born Filipino citizenship), present original Order of Approval of the parent(s).

The child need not appear during the processing of Report of Birth unless the parents want to apply for a Philippine passport for him or her, which option must be clearly conveyed through email in the booking of appointment.

NOTE: The Embassy will forward the Report of Birth (ROB) to the Philippine Statistics Authority (PSA) in the Philippines. The PSA-certified Birth Certificate (BC) may be retrieved in the Philippines after 6 months or more from the transmittal filing of the Report of Birth by the Embassy to the relevant Philippine authorities. Always keep a personal copy of the Report of Birth.
