The duration of processing of passport takes between 6 to 8 weeks from the time of application, shipment of passport from DFA-Manila up to the time of release of the new passport.  

Applicant who opted for the Mailing of Passport

Once the new passports are available, the Embassy Consular Section informs the applicants through email of the date of the mailing of their passports, with special instruction on the cancellation of their old passports.

Applicant who opted for Pick-up of Passport

Once the new passports are available, the Embassy Consular Section informs the applicants through email of the date when they may claim the new passport. On the given appointment, the applicant brings and presents their old passport to the Consular Section for cancellation.

Note: Passports left unclaimed by the applicant one (1) year after receiving notice from the Embassy Consular Section of its availability shall be cancelled and disposed.

Requirements:

       1. Accomplished Passport Application Form (Processing fee: €66.00 in PIN or cash)

       2. PSA authenticated Birth Certificate

       3. If applicable, PSA authenticated Marriage Contract or PSA authenticated Report of Marriage issued by the Philippine Embassy or Consulate in the country where the marriage took place (need not be in PSA security paper if issued by the Philippine Embassy, The Hague)

       4. Affidavit of Loss or Sworn Statement narrating the circumstances on how the passport was lost, destroyed or damaged (Notarization Fee: €27.50 in cash)

       5. Police Report, for lost passport cases

       6. If available, photocopy of the data page of the lost, destroyed or damaged passport.

The applicant shall be required to shoulder the cost of verification fee (€27.50) if unable to produce a photocopy of his/her passport.

       7. To establish identity, applicants shall also be required to submit  of the following supporting documents depending on the assessment of the application:

  • Voter’s Registration Record,
  • PRC / IBP ID
  • Baptismal Certificate
  • Land Title
  • Seaman’s Book,
  • Driver’s License
  • Income Tax Return (Old)
  • Government Service Record
  • Old SSS-E1 form/or digitized SSS-ID,
  • School Form 137 or Transcript of Records
  • Seaman’s Book,
  • Certificate of Identity
  • Tribal Membership issued by the Office of the Muslim Affairs, Counseling Certificate issued by the Commission on Filipino Overseas (CFO)
  • Other documents that show full name and birth details of applicant and citizenship)

The passport will be processed only after the 15-day clearing period (standard verification process at the DFA-Manila). If the applicant cannot wait this long, he or she may wish to apply for a Travel Document (Processing fee: €33.00), a one-way document to return to the Philippines and is recognized by the appropriate authorities in The Netherlands and the Philippines.

2. Original Philippine passport (with photocopy of the datapage)

3. A copy of and the actual Residence Permit

2.3 Minor- First-time Application

1. Accomplished Passport Application Form (Processing fee: €66.00 in PIN or cash)

2. PSA authenticated Birth Certificate or original Report of Birth (ROB) issued by the Philippine Embassy in The Hague if born in the Netherlands.

3. If the child acquired Philippine citizenship through RA 9225 by derivative means (i.e. by the reacquisition or retention of natural born Filipino citizenship by the parent/s), present original Order of Approval of the parent(s) where the child’s name is also indicated.

4. Data page of the valid passport of the parent who will accompany the minor applicant in applying for passport

5. PSA authenticated Marriage Certificate, if only one parent is accompanying the child

6. If parents are unmarried--

  • a Special Power of Attorney (SPA) executed by the Mother will be required if she is not accompanying the child in applying for passport
  • if the Special Power of Attorney (SPA) is executed in a place other than The Netherlands, it must be authenticated by the Philippine Embassy or Consulate which has jurisdiction over the place where it was executed

7. If applicant is not accompanied by the parents/s during the application process—

  • Applicant must submit a Special Power of Attorney executed by the parent/s designating the minor’s adult companion to assist in the application process. The Special Power of Attorney must be authenticated by the Philippine Embassy/Consulate if executed abroad

8. Passport or valid government-issued ID of the authorized adult companion
Original copy of proof of filiation or guardianship, if applicable 

9. A copy of and the actual Residence Permit of the parent/s or accompanying adult, and of the minor if applicable

10. School ID of the minor, if applicable

3. PSA authenticated Birth Certificate or Report of Birth (ROB) issued by the Philippine Embassy in The Hague if born in the Netherlands.

4. PSA authenticated Marriage Certificate, if only one parent is accompanying the child

5. If parents are unmarried--

  • a Special Power of Attorney (SPA) executed by the mother will be required if she is not accompanying the child in applying for a passport

2.5 Change of Name in passport

For Change of First, Last or Full Name

  • PSA authenticated Birth Certificate with annotation on the change of name

From Maiden to Married Surname

  • PSA authenticated Marriage Certificate or
  • PSA authenticated Report of Marriage issued by a Philippine Embassy or Consulate (need not be in PSA security paper if issued by the Philippine Embassy, The Hague)

From Married to Maiden Surname

  • PSA authenticated Birth Certificate of the applicant
  • In case of annulment or nullity of marriage or divorce, PSA authenticated Marriage Contract/Report of Marriage with annotation
  • In case of death of spouse, PSA authenticated Death Certificate or PSA authenticated Report of Death issued by a Philippine Embassy or Consulate (need not be in PSA security paper if issued by the Philippine Embassy, The Hague)
